Sage AWARD ENTRY FORM

Thank you for your interest in applying for the sixth annual Sage Award. If you have questions regarding the award or application, please contact Pat Bowling, Vice President of Communications for the American Home Furnishings Alliance, at 336/881-1006, or Sageaward@ahfa.us.

There are THREE PARTS to this entry form:

Part 1: Cover Page
Please provide all company contact information requested. The person submitting the entry must be an employee of the company.

Part 2: Executive Summary Do not include any confidential information in the Executive Summary.

The summary should be 300 words or less and should contain a brief description of your company’s history and primary business. Please save the details regarding your sustainable business practices for Part 3. Use the “word count” tool (found in the “tools” menu of Microsoft Word) to check your length.

Part 3: Detailed Questionnaire Part 3 of your entry form will provide the details necessary for judges to complete their final selection of the 2013 Sage Award winner. Again, do not include confidential information.

Read the questions carefully, and include only the information requested. You will have an opportunity at the end to provide additional commentary not covered by the specific questions. Any supporting documentation – such as charts showing environmental improvements, documents verifying specific environmental certification, etc., should be emailed separate from the entry form to pbowling@ahfa.us. Although this supporting documentation is not required, judges have typically looked for documentation of any environmental certifications cited in the award application.

The entry form will limit you to 1,600 words.

PART 2: EXECUTIVE SUMMARY

This summary should be no more than 300 words. Please address the following in your Executive Summary:

  1. A brief company history.
  2. Primary areas of business.
  3. Overall business goals and objectives.

This concludes Part 2 of THREE required components for a complete Sage Award entry.

PART 3: DETAILED QUESTIONNAIRE

This portion of the entry form includes questions related to three specific areas: sustainable business practices, social responsibility and profitable growth. Answers must be provided in all three sections to complete your entry.

Sustainable Business Practices.
Please describe your company’s sustainable business practices. Provide actual numbers or percentages whenever possible to document environmental improvements. Be sure to note when, how and why specific practices were instituted.

The following list may help you organize your response. You need not address ALL of the points below, but make sure your response provides a complete picture of your company’s journey toward more sustainable operations. Please limit your answer for this section to a total of 750 words or less.

  1. Organizational policies/procedures inside the organization related to sustainability, such as recycling, conservation, lower energy usage, promotion of employee carpooling, etc.
  2. Environmental impact programs and initiatives within the company’s business operations and/or manufacturing operations that are designed to positively impact the environment, such as reducing energy usage, reducing water consumption, reducing waste, using recycled components, using certified sustainable components and any other steps designed to shrink the organization’s carbon footprint. (If available, please include documentation to help judges assess the scope of these practices and send to Sageaward@ahfa.us.)
  3. Eco-friendly product design.
  4. Warehousing, shipping and/or other distribution practices that reduce the use of non-renewable energy in transportation.
  5. Supply chain practices designed to assist suppliers in the development and implementation of sustainability programs.

Social Responsibility.
Describe and give examples of how your company has encouraged social responsibility through any of the following:

  1. Corporate policies and procedures to assess and monitor social and environmental performance throughout the supply chain.
  2. Corporate community involvement.
  3. Support of individual employees' community involvement.
  4. Programs and policies that impact communities throughout the company's entire supply chain (such as in countries where materials, components or products are sourced).

Your response should include more than a list of organizations that receive your corporate donations. Please limit your answer to 300 words or less.

Profitable Growth.
Describe the savings and/or profitable growth experienced as a result of sustainable practices. Please include specific dollar amounts or percentage savings where possible. NOTE: We are not asking for confidential financial performance figures. The judges will be looking for assurances that your company is financially secure and for indications that sustainable practices are contributing to that security, not detracting from it. This section is limited to 300 words or less.
